Alteogen's Keytruda SC Expands Bladder Cancer Indication in Europe, Targeting 50,000 Annual Patients

The European Commission has approved the Keytruda subcutaneous (SC) formulation—powered by Alteogen’s ALT-B4 technology—for resectable muscle-invasive bladder cancer, expanding its treatment scope across 30 European countries to include both cisplatin-eligible and ineligible patients.

Approval extended to cisplatin-eligible patients, broadening treatment scope across 30 European countries

Additional indication gains final approval in just 34 days, expanding the commercial foundation for ALT-B4

Alteogen's (196170) proprietary subcutaneous (SC) formulation technology, applied to Merck's (MSD) blockbuster immunotherapy Keytruda, has secured an expanded bladder cancer indication in Europe. The regulatory approval now covers both patients eligible for and those ineligible for standard cisplatin-based chemotherapy. With an estimated 50,000 new cases of muscle-invasive bladder cancer diagnosed annually in Europe, the commercial footprint for Keytruda SC continues to widen.

This latest approval encompasses not only the conventional intravenous (IV) formulation of Keytruda but also the SC version utilizing Alteogen’s ALT-B4 hyaluronidase technology. As new indications for Keytruda translate directly into broader therapeutic coverage for the SC formulation, Alteogen is poised to strengthen its medium- to long-term royalty revenue base.

Expanding European Bladder Cancer Treatment Reach to Cisplatin-Eligible Patients

MSD announced on Oct. 21 (local time) that the European Commission (EC) has approved the combination of Keytruda and Padcev (an antibody-drug conjugate by Pfizer and Astellas) as perioperative treatment for resectable muscle-invasive bladder cancer (MIBC).

The approval includes Keytruda SC, which applies Alteogen’s ALT-B4 technology and is marketed in the United States under the brand name Keytruda Qlex. The regulatory decision applies to a total of 30 countries, comprising the 27 European Union member states along with Iceland, Liechtenstein, and Norway.

In June, Europe approved the Keytruda-Padcev combination specifically for patients deemed ineligible for cisplatin-based chemotherapy. With the latest expansion to include cisplatin-eligible patients, the regimen has become Europe's first PD-1 inhibitor and ADC combination therapy approved regardless of cisplatin eligibility.

The approved regimen covers both neoadjuvant treatment prior to surgery and adjuvant therapy following radical cystectomy, adding a novel immuno-oncology and ADC combination option to traditional chemotherapy.

The regulatory green light was backed by data from the Phase III KEYNOTE-B15 clinical trial, which demonstrated that the Keytruda-Padcev combination reduced the risk of disease progression, recurrence, or death by 47%, and the risk of death by 35%, compared to conventional chemotherapy. The pathological complete response (pCR) rate reached 55.8%, significantly outperforming the 32.5% observed in the control arm.

Market analysts are also closely watching the scale of the European bladder cancer market. According to MSD, approximately 208,291 new bladder cancer cases were diagnosed in Europe in 2024, with clinical guidelines classifying about 25% of new diagnoses as muscle-invasive bladder cancer—translating to roughly 52,000 patients annually.

While this figure represents the estimated total population of new muscle-invasive bladder cancer patients in Europe rather than the exact addressable patient pool for the Keytruda-Padcev combination, the target population is strictly confined to resectable adult patients.

Rapid Approval in 34 Days Broadens ALT-B4 Commercial Reach

The European approval was granted just 34 days after the Committee for Medicinal Products for Human Use (CHMP) of the European Medicines Agency (EMA) issued a positive opinion on Sept. 17.

Regulatory timelines for Keytruda SC have steadily compressed: the initial adult indication took 61 days from a CHMP positive opinion (Sept. 19, 2025) to final EC approval (Nov. 19, 2025). Subsequent approvals took 35 days for the KEYNOTE-B96 ovarian cancer indication and 34 days for the cisplatin-ineligible bladder cancer indication. The latest cisplatin-eligible bladder cancer indication matched this swift 34-day turnaround.

As recent additional indications secure final nods within roughly a month of CHMP recommendations, the therapeutic reach of Keytruda SC in Europe is expanding in rapid succession.

Crucially for Alteogen, the SC formulation continues to ride the coattails of Keytruda’s expanding label. Each new indication broadens the commercial scope of products utilizing the ALT-B4 platform without requiring the development of entirely new standalone drugs.

ALT-B4 is a proprietary platform technology designed to convert intravenous biologic drugs into subcutaneous injections. Through its licensing agreement with MSD, Alteogen has established a lucrative business model anchored on commercial royalties tied to Keytruda SC.

With Keytruda SC already entering the commercialization phase in the U.S. and Europe, its addressable patient population is growing alongside label expansions. This latest bladder cancer approval underscores how the SC formulation is capturing expanded sales opportunities not only by converting existing IV treatments but also by penetrating new therapeutic frontiers.

As actual prescriptions and sales of Keytruda SC increase going forward, they are expected to serve as a strong catalyst for Alteogen’s royalty revenue growth, marking another milestone in the commercial expansion of domestic biotech platform technologies.
